I work with a Case Management Company in Atlanta Georgia, USA for Elderly and Disabled Waiver Program (EDWP) as Case Manager for almost a year now and I believe what you are looking for will greatly fit with my experience and skills.
Our company receives referrals from the state for Elderly and Disabled Waiver Program and we are the ones responsible for Initial Assessment through RNs and LPNs and as case management company, we recommend services that will support the level of care (LOC) of the patient/client. Once they get approved from the state and will have active Medicaid Insurance, case manager will broker the client with different providers such as providers for Home Delviered Meals, Personal Support Service/Caregiving Service, Structured Family Care, Emergency Response System, and other services recommended by the nurses during the initial assessment.
I have attended training with the Area Agency on Aging on how to handle different referrals/clients as a case management company. Unfortunately, there were no certificates provided during the training since it was all intended to all Case Management Companies and CCSP Provider all over the state of Georgia.
Most of my experience for the past months were focused in a case management role. I am the one responsible to submit all brokering packets to provders to start the service. Gathering all the necessary documents and sign them for approval. Communication with patiens/clients during the brokering service is very important to ensure the frequency of the service and broker them with the client's preferred provder.
Before starting to work in a case management company for Elderly and Disabled Waiver Program, we are required to sign Health Insurance Portability and Accountability Act (HIPAA) agreement to ensure confidentail information of the client such as Social Security Number, Insurance Membership Number, Demographics Information, and other sensitive information are secured and safe.
To ensure accuracy while typing, there are tools available to make sure everything is correct and well delivered. These tools such as Quillbot and Grammarly were very useful for my work as case manager. AI tools such as CHatGPT and Gemini were also a help but only limited to diction and sentence structure.
