RN Case Manager with OASIS experience only

This is for Registered Nurses only. Non-RNs need not apply.
*Metro Manila residents are highly prioritized.

o Performing utilization and concurrent review of inpatient cases using an EMR
o Transcribing orders for clinicians as needed
o Discussing cases with treating physicians and other healthcare professionals to better understand plans of care
o Participating in rounds with the DON and/or Care Manager and discussing cases as needed
o Determining need, if any, for intervention, and discussing with Care Manager, clinicians and Physicians
o Identifying and evaluating delays in care
o Initiating a discharge planning and hospital readmission prevention plan
o Contacting and maintaining communication with acute long term care, acute rehabilitation, or skilled nursing facilities to verify patient admissions and when we can process ROC
o Gathering clinical information to assess and expedite care needs
o Facilitating timely and appropriate care and effective discharge planning. Participating in team meetings, education discussions and related activities
o Maintaining appropriate documentation
o Consulting with Care Manager as needed to troubleshoot difficult or complex cases Identifying opportunities for improved communication or process
o Hospital-experienced will be considered

MUST BE A GRADUATE OF NURSING or a REGISTERED NURSE

*Hybrid - 2days onsite, 3 days WFH (Office: Ortigas center)
