JOB OVERVIEW

VA 2: Follow-Up, Post-Surgical Care & Chart Preparation Specialist
Must be able to work Hawaii Time 8am-5pm
Need to have a nursing degree as this is a Clinical Job and need Healthcare /Nursing Skills and Knowledge

Responsibilities
Manage post-op communication and follow-up visits.

Collect feedback through surveys and digital tools.

Prepare personalized recovery plans.

Prepare patient charts with relevant notes and records.

Track patient outcomes and document notes in Elation Health EMR.

Send patients post-op instructions by text or phone call.

After each clinic visit, send patients a Patient Summary of Visit with a to-do list and due dates.

Send consent instructions via Upgrade to see actual info and follow up on cardiac clearance, EKG, and labs two weeks before procedures.

Fill out paperwork or write letters and documents that are needed by patients such as work notes, family leave , work leave, school note , etc.

Redundancy Protocol
If unavailable, VA 1 or VA 3 will manage follow-up communication and patient feedback tasks.

Scripts & Templates
Post-Op Follow-Up:

“Hello [Patient Name], we hope you’re feeling well after your procedure. Please let us know if you’re experiencing any issues or need further assistance.”

Feedback Request:

“We would appreciate your feedback on your experience with us. Please complete this brief survey: [Link].”

Daily Checklist
Send post-op follow-up messages.

Schedule follow-up visits.

Record patient feedback and update charts.

Prepare charts for upcoming appointments.

Send summaries and pre-procedure instructions.

Weekly Checklist
Review patient feedback and address negative comments.

Update the follow-up process based on feedback received.

Cross-train with other VAs for chart preparation.

Submit weekly performance reports.
