Enrollment Specialist (Bi-Lingual: Mandarin & Cantonese)

Position: Remote Enrollment Specialist

Remote Focus provides care coordination services to Medicare members with chronic medical conditions. We are seeking a Remote Enrollment Specialist to engage and enroll eligible individuals in Health Home Care Coordination services and Remote Patient Monitoring (RPM) services. This role is instrumental in onboarding patients and assisting with member retention efforts by identifying, screening, engaging, and enrolling eligible members.

Position Summary:
The Remote Enrollment Specialist plays a vital role in enrolling and onboarding patients with chronic health conditions for case management and remote patient monitoring services. This position is fully remote and requires excellent communication, organizational, and outreach skills to ensure patient engagement and program success.

Essential Duties and Responsibilities:
- Receive and process patient referrals from multiple sources, interacting with referral contacts and initiating outreach efforts.
- Create and maintain patient records in the electronic medical record (EMR) system, ensuring all necessary medical
documentation is accurate and up to date.
- Review medical documentation and Medicare-insurance eligibility to ensure appropriateness for enrollment.
- Conduct proactive outreach efforts via phone, email, or video calls to engage potential patients, family members, or caregivers,
providing comprehensive program education.
- Obtain patient consent for participation and ensure all documentation is completed and stored appropriately.
- Schedule initial comprehensive enrollment assessments for patients with the medical provider.
- Assist patients in setting up electronic medical devices for remote patient monitoring and provide necessary technical
guidance.
- Perform a warm handoff to the assigned Case Manager once the patient is officially onboarded.
- Work closely with internal teams to support member retention and enhance patient experience.
- Perform other related duties as assigned.

Required Skills and Experience:
- Minimum 1 year of experience in medical sales, patient enrollment, or care coordination, preferably within medical or
behavioral health settings.
- Strong interpersonal, communication, and teamwork skills.
- Proficiency with electronic medical records (EMR) systems and remote patient engagement tools.
- Ability to work independently in a remote environment while meeting outreach and enrollment targets.
- Strong organizational skills to track and follow up with potential enrollees efficiently.

Bilingual in English/Russian preferred.
